Eligibility: Referral form in required. Must meet state-regulated eligibility criteria.

Intake Procedure: One a completed referral is received, Penn Foundation will contact the referred participant to schedule an intake in his/her home or somewhere in the community, wherever her/she is most comfortable. The case manager will complete an assessment to determine if he/she meets the criteria for case management and what lever of care is needed. Once a supervisor approves the case manager's assessment, registration paperwork is completed.
