Mental Health Case Management
Mental Health Case Management focuses on community integration and the life domains of housing, financial benefits, education, employment, physical and mental health, and natural social supports.

There are two levels of support:
A) Administrative Case Management: Primarily office-based and less intensive; available to children and adults of Bucks and Montgomery Counties
B) Blended Case Management: Primarily community-based and offers a more intensive level of service; available to children and adults of Bucks and Montgomery counties; recovery coaching is a model of Blended Case Management for adults that offers team-delivered services with a clinical consultation component

Hours: Monday through Thursday, 8:00am to 8:00pm

Friday, 8:00am to 5:00pm
