Contact local VA Hospital or Clinic, County Veterans Office or
similar Veterans Service Agency. The social worker and caseworker
develops the best course of action.
Intake Requirements:
Bring these documents to your appointment:
• Proof of Pennsylvania residency
• DD 214 (Discharge)Service-connected disability award letter (if you have one)
• Copies of lease and other bills you need assistance with. (Ensure they have addresses and account numbers on them.)
• If car repair, also bring copy of car title, car insurance and car registration.
