Intake Procedure:
Order in person at a Pennsylvania Vital Records branch
office in Erie, Harrisburg, New Castle, Philadelphia or Scranton.
Delivery ranges from same day to five days based on teh public
office processing time.

The application processing time is expected to take approximately three (3) weeks.
- Application Information: (NOTE: Do a test "save" of the
application before you complete it. "Printing to file" seems to
work better than just trying to do a "save as".)

- An advocate (shelter staff, social worker and/or attorney) must also fill out their portion to on the application, include photocopy ID and a letter on agency letterhead.
