Chambersburg - Fifth Avenue - LIFE (Living Independence for the Elderly)
Offers preventive, primary, acute, and long-term care services. Provides the following services: day services and recreational therapies, primary medical care and specialty services, laboratory and diagnostic services, vision, dental, hearing and foot care, prescription drugs and other medical supplies and equipment, nursing, physical therapy, occupational therapy and speech therapy, social services and behavioral health care, home health care and personal care, nursing home care, acute hospitalization and ambulance services, 24/7 access to emergency/urgent care, nutritional counseling and meals, and transportation.
Eligibility: A) Adults age 55 and older who meet the clinical eligibility needs for a skilled nursing facility
B) Meets financial requirements as determined by the local county assistance office or be able to pay privately
C) Resides in an area served by a LIFE community at enrollment as determined by a LIFE provider
D) Agrees to have all services coordinated and approved through the LIFE interdisciplinary team
Hours: Monday through Friday, 8:00am to 4:00pm; On Call Nurse: 24/7

Helpful Tips: Services are not limited to those offered at our life centers. Provides home care and in-home support, as necessary, to meet each participant's plan of care. These services may be provided in the morning to assist the participant in preparing for a life center visit, days the participant does not attend the life center, evenings, or weekends.
