Penndel Mental Health Center - Bucks; Langhorne - Projects for Assistance in Transition from Homelessness (PATH)
Provides assistance to eligible individuals who are homeless or in danger of becoming homeless and who also have severe mental illness. Meets with clients in the community and helps clients locate housing (temporary or permanent), financial assistance to avoid eviction, and links to mental health and substance abuse treatment.
Eligibility: Persons experiencing or at risk of homelessness with severe mental illness

Fee Amounts: There is no fee for PATH services.
Intake Requirements: All individuals interested in the utilization of PATH Outreach Services are highly encouraged to first contact Housing Link at 1-800-810-4434. Housing Link will then submit referrals to Penndel Mental Health Center’s PATH Program to conduct homelessness site verification.
