Gives eligible individuals extra support to live successfully in the community. Services include supportive case management in conjunction with recovery/resiliency oriented principles.
Eligibility: Must be diagnosed with a mental illness or have a history of needing special services.
Hours: Office hours are 8:30AM - 4:30PM.

Intake Procedure: Referrals are typically made by a mental health professional and require a psychiatric or psychological evaluation within 12 months of treatment.
