Provides tens of thousands of appointments each year for individuals living with muscular dystrophy, ALS, and other neuromuscular diseases. Offers access to expert multidisciplinary care, clinical trials, and opportunities to connect with MDA and the neuromuscular community.

Intake Procedure: Call the Resource Center at 1-800-572-1717. A Resource Specialist will help you locate your local Care Center based on your zip code. You will then reach out to the chosen location regarding their intake process.
Intake Requirements: Doctor referral generally required as well as related medical records
