Central's Case Management Services are provided through various levels of care which include Intensive Case Management (ICM), Resource Coordination (RC) and Service Case Management (SCM). These programs are designed to reduce homelessness and hospitalizations of persons suffering from severe and persistent mental illness.
Eligibility: Individuals experiencing homelessness and symptoms of mental illness
Hours: Monday through Friday, 9:00am to 4:00pm
