Aging Waiver Program
Develops and implements creative alternatives to placing Medicaid-eligible individuals in medical facilities such as nursing homes and Community Residential Facilities (CRF). Recognize that many individuals who are at risk of being placed in a medical facility can be cared for in their natural homes and communities through the use of home and community-based waiver programs.
Eligibility: Must be a resident of Pennsylvania, be a U.S. citizen or a qualified Non-citizen, have a Social Security Number, be 60 years of age or older, meet the required level of care for a Skilled Nursing Facility and meet the financial requirements as determined by your local County Assistance Office.
Hours: Monday through Friday, 8:30am to 4:00pm
