Lower Bucks Acute Care Center - Assertive Community Treatment ( ACT)
Team of skilled professionals that give intensive support to a mental health consumer in the community, including psychiatry, nursing, social work, case management, substance abuse counseling, peer support and supported employment.
Providing comprehensive community based treatment for persons with severe and persistent mental illnesses
Eligibility: A) Have a primary diagnosis of schizophrenia,
schizoaffective disorder or bipolar disorder
B) Have a history of recent hospitalizations,
incarceration, homelessness, co-occurring
disorders, and/or legal issues
C) Are eligible for Medical Assistance
D) Are unable to benefit from less intensive
levels of care (i.e., case management,
outpatient services)
Hours: Monday through Friday, 8:00am to 9:00pm
