D) Eligible for Magellan Insurance or have Medicaid, Private Insurance, or no Insurance (will qualify for County Funding)

Referral form needs to be filled out by a Licensed Practitioner of
the Healing Arts. (physician, Physicians Assistant, Licensed
Psychologist, or Certified Registered Nurse Practitioner) Call for
information on how to obtain the form.
