Children from birth to age 3
Have a 25% delay in one area of development, a medical diagnosis that may result in a developmental delay, or a qualifying clinical opinion

Intake Procedure:
To make a referral call or email.. Service coordinators schedule
with families/caregivers to meet them at home or in a community
location of family choice for the initial needs assessment. They
discuss family concerns and desires for their child's development.
They work with family to plan initial evaluation for eligibility.
Intake Requirements:
This program is voluntary. If referral is coming from outside the
family, family must be in agreement.
