Horizon House - Delaware County - Assistance in Transitioning from Homelessness
Engages and coordinates services and resources to meet the needs of persons with mental illness who are experiencing homelessness. Collaborates with clients to assess their needs and preferences to assist with community integration. Works in conjunction with mental health specialists, community supports, and housing resources to facilitate a transition from homelessness to community living.
Eligibility: Serves Delaware County residents who are experiencing homelessness and mental illness.
