Dental Lifeline Network - Pennsylvania - Donated Dental Services
Provides free dental services for older adults, individuals with disabilities, and those who qualify as medically fragile.
Eligibility: Applicants must lack adequate income to pay for dental care, and meet one of the following criteria:

A) Have a permanent disability

B) Be age 65 or older

C) Qualify as medically fragile

Fee Amounts: From time to time, people who can pay for part of their care may be asked to do so (for lab work, etc.)
Intake Procedure: Apply online or print and return a completed application and required documents by mail.
Intake Requirements: Proof of income and expenses, insurance information, dental and medical histories and current issues.
Geographical Area: Service available across Pennsylvania; wait list and providers vary by county.
