Supports Coordination through WCSI's Support Coordination Organization (SCO) is a critical service that involves the primary functions of locating, coordinating, and monitoring services and supports for consumers of all ages with an Intellectual Disability or Autism Diagnosis with onset prior to the age of 22. Support Coordinators inform participants, their families and other caregivers about the use of unpaid, informal, generic, and specialized services and supports that are necessary to address the identified needs of the participant and to achieve the outcomes specified in the developed Individual Support Plan (ISP).
Eligibility: Individuals with a mental health diagnosis.
Hours: Monday through Friday, 8:30am to 4:30pm

Intake Procedure: Call, or drop-in completed through Westmoreland County Intake Office for Support Coordination.
Intake Requirements: Documentation that includes recent evaluation or assessments. Documents must indicate a diagnosis of intellectual disability or autism spectrum disorder prior to the age of 22.
