Provides supports coordinators and case workers in the community to meet their clients and help them accomplish their life goals.

Supports individuals with intellectual and developmental disabilities by helping them obtain services to live to their fullest potential. Links people in Washington County with a mental health diagnosis to needed support services and resources within their community.
Eligibility: Any person with a mental health diagnosis or an intellectual and/or developmental disability within a 45-mile radius of the site's location in Washington and Allegheny County.
Hours: Office Hours: Monday through Friday, 8:30am to 5:00pm
