Eligibility:
To be eligible, one must be the primary caregiver for an adult age
60 or over or an adult with chronic dementia. Primary caregivers
for an individual, ages 19-59, with a disability and relatives
providing care for children may also qualify for the program.

For more information regarding the Caregiver Support Program,
please call 800-734-9603. To make a referral, visit the intake
website.
Once a referral is made, an assessor will visit the home and complete an assessment of caregiver needs, care recipient needs, and the home environment.

Helpful Tips:
In addition, assistance may be available to caregivers 55 years of
age and older who reside with a relative adult with disabilities
aged 18 to 59 who is not the biological or adopted child of the
caregiver and is not receiving Medicaid Waiver services.
