Mercer - Caregiver Support Program
Assists families in assessing their needs when taking care of older relatives.
Services include some financial support, benefits advice and counseling where caregivers are advised of benefits that are available to them.
Eligibility: Primary hands-on caregiver to person who is 60 years or older who is a resident of Mercer County and meets specific Physical/Health measurements.
Hours: Monday through Friday, 8:30am to 4:30pm

Intake Procedure: Assessment of needs, completed after initial intake.
Intake Requirements: Senior consumer's Name, Date of Birth, Social Security Number, Income information, Address, Phone, Physical/Health Info, Caregiver's Name, Address, Phone, Social Security Number.
