Administrative Headquarters - Blended Case Management
Links adults and children who suffer from serious mental illness or emotional disturbance to necessary community resources.
Eligibility: To qualify for services, individuals must be diagnosed with a psychiatric disorder, receive Medical Assistance, and fit one of the following categories:

1) Have spent 6 or more days in psychiatric inpatient treatment

2) Met criteria for involuntary treatment

3) Missed 3 or more mental health service appointments

4) Have had 2 or more face to face visits with the Mobile Crisis Unit

5) Is receiving, or in need of, services from 2 or more human service agencies
Hours: Monday through Friday, 8:00am to 4:00pm
