Administrative Headquarters - Community Health Worker Program
Provides a program that assists individuals with bridging the gaps between their needs and available resources. Community Health Workers can meet with clients in a variety of locations to determine the current barriers that are impacting an individual's overall health and wellness. Community Health Workers will help individuals to create a plan with goals and action steps address these barriers.

Eligibility: Adults 18 or older who have behavioral health needs, or co-occurring substance use and complex needs who are residents of Westmoreland County.
Hours: Monday through Friday, 8:30am to 4:30pm

Intake Requirements: Self-referrals are encouraged; referrals from other sources are also accepted.

If arriving in-person, ring the doorbell outside of the office entrance on the second floor.
