Assists eligible individuals and families to address risk factors of for their ability to lead a healthy lifestyle and to retain a stable, health home environment within the community.

Help is available for childcare, clothing, food, shelter, hygiene products/household goods and supplies, utilities, and transportation that is not covered by the Medical Assistance Transportation Program (MATP) such as bus passes to go to the grocery store, work, recovery meetings), and more.

Eligibility: Adults age 18 and older who have behavioral health needs and live in Westmoreland County.
Hours: Monday through Friday, 8:30am to 4:30pm

Intake Procedure: Call or email to schedule an assessment and intake appointment

Appointments will determine if eligibility requirements are met and will discuss guidelines for receiving funding.
Intake Requirements: Documentation of need is discussed at first phone contact with community health worker.

Social Determinants of Health (SDoH) are non-medical factors that influence health outcomes. These are conditions in the environment in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality of life outcomes and risks.
