Intake Procedure: Call to schedule an intake appointment. Leave your name, phone number, zip code, and ages of your children.
Intake Requirements: Valid photo ID, proof of all household income, current photo of child, and proof that child is living with you. Also include child's clothing size and wish list.
Geographical Area: Serves 32 Zip Codes in western Fayette County. Provides referrals to other sites for individuals outside the service area.
Helpful Tips: Apply by the end of October.

Office is in the lower level of Saint Francis of Assisi Catholic Church. Entry is at the rear of the Saint Benedict Learning Center.
