Pittsburgh - Case Management Program
Offers expanded bilingual service coordination and case management to families needing support. This includes working with dedicated case managers who partner collaboratively with families to arrange, coordinate, monitor, evaluate, and advocate for services tailored to individual families' needs.
Eligibility: Families with complex needs requiring ongoing regular support for intensive needs.

Intake Procedure: Call 412-335-7446 and select option 1 to connect to an Intake Specialist.

The Intake Specialist will complete an intake form over the phone (about 10 to 20 minutes), then refer to a Case Manager.
