Shady Avenue - Supports Coordination
Supports Coordinators assist individuals age 3 and older who are diagnosed with an intellectual/developmental disability or autism, before the age of 21, to attain necessary supports and services to remain safe and to increase the quality of their lives.
Eligibility: Individuals age 3 and older who are diagnosed with an intellectual/developmental disability or Autism

Intake Procedure: Individuals are referred through each county Administrative Entity. Referrals do not need to be Allegheny County based. Call for details.
