mobile or semi-mobile, but cannot live alone.

Intake Procedure:
Call to request intake. The Dom Care Department will contact
consumers and prospective providers to screen for eligibility and
to begin the application/matching process.
Intake Requirements:
Information about the individual seeking to enter Dom Care (date of
birth, Social Security Number, disability information) is required
for initial intake. Details of other requirements provided during
the screening process.
