Manages many of the day-to-day responsibilities associated with coordinating support services for individuals with Waivers.
Eligibility: Individuals age 18 or older with a Participant and Family Directed Services (PFDS), Community Living Waiver, or Consolidated Waiver.
Hours: Monday through Friday, 7:30am to 3:30pm
