Accepts most third-party payments including state Medical Assistance plans and private insurance. Sliding scale fees are available for people without insurance.
Eligibility: No restrictions.
Hours: Monday through Thursday, 8:45am to 5:00pm
Friday, 10:00am to 5:00pm
